How many printed copies of Easy Read complaints or feedback resources, if any, have been produced by Health New Zealand in each of the last three financial years, broken down by year and region or district?

I am advised that Health New Zealand does not hold any central record relating to the production of Easy Read complaints and feedback forms, as thse forms are printed by individual service units as needed. I am further advised that Lakes District's records show it has produced the following number of Easy Read forms: • 50 in 2023/24 • 2030 in 2024/25 • 710 in 2025/26
