Was there any incident at Korowai Manaaki whereby young people were lined up on two sides and a young person walked through the two lines while he was hit; if so, was a serious event notification made about the incident?

Answer

Subsequent to my answer to WPQ 34057 (2026), I have been advised by Oranga Tamariki that a Serious Event Notification (SEN) was not completed in this instance, however it was escalated to senior leadership through an incident briefing process. I am further advised that a SEN is a notification tool, but it is not always mandatory to use and is not the only mechanism available to notify/escalate incidents, in particular where the matter may be more historic or there is no immediate risk of harm identified. In this case, I am advised the residence and the National Support Services team used an incident briefing process, which provided largely the same information as a SEN would.
