  • Has the Minister been advised whether structured post-incident debriefs were undertaken with frontline staff following the Auckland and Southern hospital IT outages in January 2026, and if so, what risks or concerns, if any, were identified?

Answer

I am advised that post-incident reviews, which are a standard operational process following an outage, include engaging with hospitals and staff to capture their assessment of the impact of outages. This is part of finalising a post-incident review so that Health New Zealand can work on reducing the impact of any future incident.
