How do Police measure whether IPCA investigations lead to systemic improvement rather than case-specific correction only?

Answer
I am advised that Police implementation of Independent Police Conduct Authority (IPCA) recommendations is overseen through Police governance arrangements to ensure actions result in sustained organisational improvement rather than one off case corrections. Recommendations and proposed closure actions are discussed with the IPCA, and no recommendation is marked as closed without IPCA review and agreement.
